To make the form quicker to fill out, some of the more common answers are already checked - be sure to change them if appropriate for your dog.

The form has places for anything that you want to tell people about the dog. To save yourself from typing the same thing multiple times, please be sure you are answering only what the question asks for.

If you are listing more than one dog, please fill out a form for each.

Because dogs with pictures seem to find homes faster, we would like one or more photos if at all possible. Please send these as quickly as you can. (Instructions for sending photos are included in the follow-up page after you complete the form and in the e-mail confirmation you will receive.) Please include the dog's name with the picture, so we can match it to the correct profile.
